From the South: I-295 North to exit 40 (for Route 38) toward Mount Holly. Continue 6 miles on Route 38 East. Make the jughandle at Main Street to turn left onto Madison Avenue (towards the hospital). Rehab Connection will be on your right.

From the North: I-295 South to exit 47A (CR-541 South) toward Mount Holly. Merge onto Burlington-Mount Holly Road which becomes High Street. Turn right onto Washington Street. Turn left onto Madison Avenue and Rehab Connection will be on your left, past the hospital.
